I don't think an official reason has been released, but it sounds like a combination of pain killer addiction, his own pride, and the medical system failing him. He really should have gotten some real help the week prior when he almost died. He would very likely still be alive had he received the proper help then.

A 30 year opioid addiction finally caught up with him after he added a shitload of Percocet on top of his regular regimen of surgical/palliative narcotics. Amazing what one gets away with when they got money.
