Probably did catch some civilians, but it also probably crippled the ISIS "facilities" near Tora Bora as well as Jalalabad. Little tactical bombs wouldn't do much of anything to the compound, but the MOAB explodes moments before impact, causing pressure that collapses places just like below:

They steal oil in the Middle East and sell it illegally. Stopping that would require a physical presence in the Middle East deterring them from stealing oil.
